Honoured to be invited to speak at IMA Pune – MULTICON 2025, held recently on 6-7 dec 2025, on the topic “Paradigm Shift in the Management of Hernias and Abdominal Wall Reconstruction.”
Over the past two decades, Herniology has seen a major transformation—from conventional open repairs to anatomically precise, minimally invasive, and technology-driven reconstructions.
My talk highlighted how advances in abdominal wall anatomy and myofascial understanding have reshaped surgical planning, refined mesh-plane selection, and enabled techniques such as eTEP, Ventral TAPP, PeTEP, TARM, and TAR, significantly broadening the scope of minimally invasive surgery.
Developments in anaesthesia, including regional blocks, multimodal analgesia, and ERAS pathways, have improved perioperative recovery and reduced pain.
Robotic surgery remains a true disruptor, offering superior dexterity, visualisation, and ergonomics—bringing greater consistency and reproducibility to complex AWR.
Adjuncts such as botulinum toxin, preoperative progressive pneumoperitoneum, and fasciotens now allow safe repair of massive hernias once considered inoperable.
The convergence of anatomical insight, advanced technology, and optimised perioperative care has ushered in a new era of safer, more effective, and patient-centred hernia management.
